Using directional terms, etc...

You are an emergency medical specialist and you are brought to the scene of a major car accident where there are 6 people who were involved in the accident with varying degrees of injuries ranging from minor to critical. Your job is to prepare a report for the emergency room that describes the injuries with body regions and directional terms.

There was an accident on Highway 14, coming off of Highway 67. 3 high school basketball players, Gordon, Spike, and Pat, were edging out into the intersection in the driver’s purple Kia Juke to see around the cornfield. They were hit by a Dodge Caravan, containing 3 elderly women on their way home from Poker Night at Golden Years.
In the first car, we have 3 male victims, all high school students between the ages of 15 and 18. The male driver of the Juke sustained the most serious injuries, as the caravan hit the car where he was sitting. He has severe bleeding from his thigh and his leg appears to be broken in several places. He also received a blow to the head and has been in and out of consciousness. His passengers sustained relatively minor injuries. The male in the passenger seat experienced a whip-lash effect from the sudden impact, and is complaining of pain to the neck, chest, and lower back. The back-seat passenger sustained relatively minor whiplash and is complaining about pain to the neck and chest from a seat-belt laceration. The elderly women in the Caravan are mostly shaken up, but are being taken to the hospital as a precaution. The driver complains of slight wrist pain from where she hit the steering wheel. Otherwise, the women are being sent to the hospital to check for head injuries.
The driver has massive bleeding from the left femoral region. There appears to be several breaks in the left crural region, and the victim sustained an injury to the cephalic region. He has been in and out of consciousness. The passenger is complaining about pain in the cervical and lumbar regions. He sustained severe bruising in the thoracic region. The cephalic region should also be examined to rule out any head trauma. The final passenger sustained injuries to the brachial and thoracic regions, but they appear to be minor. The 76-year-old driver of the second car has a possible break in the brachial region. She and her two passengers should be observed for cephalic trauma.

The scene is a house on Rockwell Street, house number 254. There has been a case of domestic violence and child abuse—one woman and five children. The perpetrator has fled the scene. At the time of discovery, the woman had been hit in the head with a lamp and is unconscious—also has wounds on the back. The children are coherent, but some are in critical condition. Child 1 has scratches on the neck, chest, and forearms from a fork. Child 2 has bruises on the thighs and calves. Child 3 has been stabbed in the abdomen, and Child 4 has a mirrored wound—these two children are twins. Child 5 has a broken arm from riding his bike on the way to rescue the scene.

Woman, female, 42 yrs.—Posterior trauma to the cephalic region, possible concussion. Gouges lateral on the lumbar region.

Child 1, male, 16 yrs.—Lacerations on the cervical, thoracic, and antebrachial regions. Cuts are superficial on cervical, and superior, and are deep on thoracic and antebrachial regions.

Child 2, female, 12 yrs.—Multiple anterior contusions on the femoral and sural areas—possible breakages.

Child 3, female, 10 yrs.—Puncture located in the RLQ of the right hypochondriac region, piercing the cortex of the stomach.

Child 4, female, 10 yrs.—Puncture in the LLQ of the left hypochondriac region, very deep, perhaps into the pelvic cavity.

Child 5, male, 7 yrs.—Fracture in the brachial region—distal from the axilla.

There are six people that were involved in this car accident: Two adults in one car and one adult and three children in the other. The children appear to be around ages 2, 8, and 15 respectively. The car with two passengers braked unexpectedly to avoid hitting a dear and the second car hit it, causing the first car to slide down a slope off the side of the road and land upside down. The younger children were wearing seatbelts and don’t seem to have any major damage. The 15-year old hit his head on the windshield and hurt his wrist trying to block himself. The mother of the children, who was driving the car with them in it, flew halfway through the windshield on impact and has many cuts and a possible fracture to the head. The two adults, one woman and one man, who were in the flipped car are in critical condition and are being rushed to the hospital in helicopters while the other four are traveling in ambulences.

Report:
• The man has broken bones in the femoral and brachial regions.
• The woman driving with him has deep cuts in the thoracic and abdominal regions. She also has broken or fractured bones in the lumbar region.
• The 15-year old has many peripheral cuts, along with broken bones in the antebrachial and manual regions. His right hand got shoved back toward his body so it is now proximal to the other hand.
• The other two children have only superficial cuts and bruising of the cephalic and thoracic regions.
• The mother has lost her left eye, has a deep cut to the cephalon and possible fractures to the cervical region. There are also small cuts lateral to the eyes.

We arrive at the scene just before dark, the largest bank in Wisconsin, we investigate the area and a few detectives run off to go question the witnesses. The six victims were quickly but carefully hauled off in the ambulance and started to begin examination. I was lucky enough to talk to the detectives before I had to examine the wounded. The detectives stated that there was a bank robbery and that three robbers came in at 6:30pm and fired twelve times, shooting four people. The remaining two victims were fortunately not hit by any bullets, but since they refused to hand over any money, the robbers physically beat them till they gave in. Once I arrived in the ambulance I looked at the people’s information on their condition
Victim 1- shot deep in the right sagittal region, toward the heart. Serious condition
Victim 2- shot nipped the left ear lateral to the eyes. Scrapped the right elbow, proximal to the wrist, when reacting to the shot.
Victim 3- shot in the left upper quadrant, however bullet is superficial
Victim 4- shot in the abdomen leaving injuries deep and cortex.
Victim 5- pain in neck, or cervical area from being straggled. Should also be observed for thoracic pain.
Victim 6- broken finger, distal to the left wrist. Has an inferior fracture at the ankle, toward the pedal.

As we pulled up to the accident in the ambulance, it was apparent that a 2 person car with 6 passengers occupying it had run off the road into a tree. Nothing was left of the car, there was glass everywhere, with the 6 people scattered around the wreckage. The first victim we encountered was a 26 year old male; he suffered only scrapes and bruises. The Second Victim we encountered was a 42 year old female; she suffered from a severe head laceration, and a severe concussion. The third victim we encountered was a 65 year old male; he had multiple lacerations and a broken left arm. The fourth victim who is a 39 year old male went through the windshield and was impaled through the abdomen by a tree limb. The fifth victim who is 55 years’ old, suffered severe head and neck trauma. The sixth victim suffered a broken left leg and cuts and bruises of the chest.
Victim 1: minor scrapes and bruises of the cervical thoracic and abdominal regions.
Victim 2: severe head lacerations and a severe concussion of the cephalic region.
Victim 3: severe lacerations of the cervical, thoracic, abdominal, pelvic and lumbar regions; broken left antebrachial.
Victim 4: impaled abdominal region from tree limb, critical condition.
Victim 5: severe cephalic and cervical trauma

There were 6 people involved in this car accident, two cars heading south on I-90 this morning. The truck in front of the small red Grand Prix hit their breaks unexpectedly, no reasoning behind it, there were no witnesses to tell whether or not a deer had ran in front of the vehicle.
The Grand Prix ran underneath the truck causing major damage, the windshield was broken in causing glass to go into the driver and front seat passenger’s eyes, and the driver has been reported with a broken nose. The three little kids in the back suffered from broken wrists, and arms, no major injuries reported. The travelers in the truck suffered from broken ribs, and some minor bruising.
- The driver of the Grand Prix suffered from some injuries in the Cephalic Region.
- The 5 year old, one of the children in the back, suffered a broken Ante brachial.
- The 7 year old suffered a broken Brachial.
- The 21 year old in the passenger seat has a broken Nasal Region.
- The driver of the truck suffered from broken ribs in the abdominal area.
- The passenger in the truck suffered from some bruising in the Cervical and Thoracic regions.

I have just arrived at an accident that happened in the middle of an intersection. There are six people injured from the accident. Three are adult aged females around 30’s. The other three are males ranging from 30’s-40’s. The youngest female with the brown hair has a major blow to the head and is now lying unconscious. The dark brown female also in her mid-thirties has a piece of metal going through her right shoulder right above her armpit. She is awake but in pain with slow bleeding removing the metal will cause faster bleeding. The oldest female with the blond hair has scraped her arms right and left. The left one being far worse may need stiches. We have a male long blond hair with no movement probably because his head went through the windshield of his car hitting class going through his brain. We have a black haired man being held in place it seems to be that he has broken or fractured his lower back. He is having trouble staying awake. The last man at the accident is not hurt that bad he has a piece of glass in his thigh. The hardest part will be getting all of it out. All of them are in major trauma and have been rushed to the hospital that I work for.
The victims have been identified and I was responsible for talking to the family members. I talked to brown haired family first the youngest of the three females her name was Emily. “Emily will be fine she has hit her Cephalic hard this causing her to go unconscious, she has woken up though an hour ago. The blow was posterior of the cephalic. She will be ok and be able to come in a day.”
The second family that I talked to was with the dark brown female named Anna. “The metal was superior to the axillary and it was removed. This caused major bleeding but was stopped. We sewed wound and she will be fine. Movement in Brachial is a little slow though.”
Third family that I talked to was the parents of Christiana the blond female. “Her antebrachial was badly scraped minor bleeding. She was required for no stiches. They were manly superficial which was a good thing.”
The long haired male, Ben which had died during the accident was the next family I was going to see. It was the hardest and toughest one to say. “Your son was not wearing his seat belt and when the impact of the other car hit he went through the windshield. Glass ended up going through his ventral cephalic. He died instantly. No pain was involved.”
Then was Alex’s family, the black haired man he had suffered the second most but lived. “Alex had twisted his lumbar in in the umbical region. The results of this will cause him to never walk. His nervous system was cut off at the transverse. Meaning his bodies inferior parts won’t work anymore.”
The last man that I saw at the accident was Greg and his family was getting impatient from waiting. “Your son is fine her suffered minor deep wound to his femoral. It was caused by glass and all of it was removed. He needed stiches and pain medicine walking will be hard also but he is ok.”

Big Cat was driving Big Baby, Jordan, a girl with a big forehead, Big Booty, and a kid with little hands to a Bucks game on Friday December 13th, 2011. They were driving up I-43, eating the McDonalds that they just got in Mukwonago, when a pickle from Big Cat’s McDouble fell onto his lap. He was wearing his brand new LeBron James Bucks jersey since he was just traded. Big Cat freaked out and let go of the wheel to try and get the pickle off before it stained. The kid with little hands tried to grab the wheel but his hands weren’t big enough, and the car swerved off the road, flipped about four times and went through a steel farm gate. When emergency vehicles rushed to the scene Jordan was standing outside the vehicle while everyone else was trapped inside. Jordan managed to crawl out the car with only a few scraps on his chest. Big Booty had to be pulled out of the car but she had a pretty large gash on her booty. Big Baby was the only other one that had only a few minor injuries. He had a slight concussion, and also possibly a separated shoulder. Big Cat, the girl with a big forehead, and the kid with little hands had to be transported by helicopter to the hospital. Big Cat had a broken ankle and extreme pain in his lower stomach. The girl with a big forehead had a large piece of skin missing in the shape of a circle in the middle of her forehead. She also had a broken wrist and something wrong with her lower back. The kid with little hands had to be rushed into surgery. Both his hands had to be cut off and he had a severe neck injury.

On a cold, snowy winter night we got called to an accident where a minivan slipped off the road and hit a telephone pole .Once we got to the scene we saw that there were six people were severally injured in the van. Since the van was going too fast for conditions the van had slipped off the road and hit the telephone pole head on. This caused the air bags to come out and the four children in the back weren’t buckled in which caused them to go through the window. The two adults that were sitting in the front seats each had a broken nose and a major concussion. On the other hand, the four children in the back of the van were seriously injured. Child 1 was bleeding severally from the head which was causing him not to respond. Child 2 had a fractured skull and a broken neck. Child 3 had many bruises on her thighs. Child 4 had a broken femur and ankle. The child 1 and 2 were rushed to the hospital in helicopters, while the mom, dad and other two children were transported by ambulances.

Mom – Fractured nose, medial to the eyes and concussion in the cephalic region.
Dad- Fractured nose, medial to the eyes and concussion in the cephalic region.
Child 1- multiple lacerations in the cephalic region.

It is 10:07 on January 13; one of the iciest nights we’ve seen this winter. I am an EMT headed toward a major crash site in Walworth, Wisconsin on six corners road where there is a reported six people injured and a total of 4 cars. Three people in one car, two in the second car, and one in the third car. When I arrive on scene, it’s a horrid sight. Cars are piled up and there has been a head on collision. People are stuck in their cars screaming and crying, while some are unconscious lying outside or hanging out of the cars. One lucky girl suffered from very minor cuts and bruises while some injuries were more serious; one young man broke his arm, wrist and fingers. Two of them have severe concussions, a cracked skull and one has a broken nose. One unfortunate man has a large shard of glass from the windshield sticking out of his thigh and a broken femur. There was also a broken collar bone and a few broken ribs. They have been transported to the ER and are being treated.
Hospital Report:
Victim 1- Sarah Jenkins, age 26 appeared to be unharmed but has a few minor cuts and bruises on her upper right Thoracic Region.
Victim2- Ian Samuels, age 14, broke his upper left brachial region, his left wrist inferior to his Brachium and superior to his manual, and 3 phalanges.
Victim 3- Brooke Newman, age 46, is suffering from a major concussion in her Cephalic Region, a cracked Cephalon, and a broken nose.
Victim 4- Andy Cortez, age 19, has a large shard of glass sticking out from his Right Inguinal Region and a broken left Femoral.
Victim 5- Brian Whistler, age 48 is suffering from a broken Thoracic (collar bone) Region and 3 ribs on his Upper Left Abdominal Region.
Victim 6- Morgan Parks, age 21 is suffering from a large cut in the back of her Cephalic Region, a severe concussion, and a chunk of her right Sural Region has been ripped out.

I walk onto the scene and it is unlike anything I’ve ever seen before. It horrifying an eighteen wheeler slid off the icy road crossed the median on the interstate and collided with a bus of deaf and blind second graders on their way home from the zoo. Not too many people are seriously injured. The first person I see is the driver of the truck who was flung from his vehicle. He complains of a pain in his thigh. I check it out and realize that he has a cut on his thigh, but further down I see that he has a piece of metal imbedded in his calf. I leave him to check to see how many others are injured. I have to open the top hatch of the bus to enter because it has flipped onto its side. I see that many of the kids are just shaken up so I have one of the chaperons help me gather up the children who aren’t hurt and get them off the bus. Now there are only groans and sounds of agony coming from the bus. I start by looking at the children. On has a broken forearm from when he tried to brace himself. I gingerly remove him from the being careful not to touch his arm. Then the chaperon calls me over to look at a little girl that she is worried about. It turns out this girl was sitting where the truck struck the bus. She is unconscious and has several broken bones and is bleeding from the side of her head. I decide to leave her still until they can cut through the side of the bus to remover her more safely. I move onto a little boy who I think has a broken pelvis; he is by the door so we remove him right away. Next I see another little girl, she has a bar from the bus seat sticking out of her abdomen; we carefully get her off the bus and move on. Finally I help the teacher who has several broken bones and cuts from the windshield’s glass.

Bio 2: accident


Victim 1- he was able to walk away from the accident with just a few scratches on his forehead.
Victim 2-he only suffered minor injuries with a cut on the side of his head and a broken wrist.
Victim 3-she had a deep cut in her right leg and a broken collar bone.
Victim 4- he had a broken nose and a concussion. Also, a piece of glass stuck deep into his left shoulder.
Victim 5- she was unconscious with a fractured skull, crushed right leg and both broken forearms.
Victim 6- he had a broken lower back, both broken legs and one broken arms.

I arrived at the accident on that snowy day. What I saw there wasn’t pretty. 6 young adults looking to be all in their early 20’s got in a head on collision. Involved in the crash was a very large F-350 truck and a very small Toyota. There were 2 males and 1 female in the truck, and 2 males and 1 female in the Toyota. The young people in the truck had significantly smaller wounds than the poor kids in that little car. They all were transported to the ER and are being treated now.
The hospital report:
Victim 1- he is perfectly unharmed despite the insignificant scrapes and scratches on the anterior part of his cephalic region.
Victim 2- this young man suffered a deep gash on the lateral right side of his cephalic region that required 10 stitches.
Victim 3- she suffered a relatively deep inferior cut on her right crural region that had to be bandaged. She also has a shattered superior right collar bone, inferior to the cervical region, but superior to the thoracic region.
Victim 4- he has a medial injury to his nose that was shattered by the impact, and a superior concussion to the cephalic region of his body. Also, he has a piece of glass stuck deep within the right upper quadrant of his abdominal region.
Victim 5- she is still unconscious and has a fractured cephalon. Her right crural is shattered in the femoral region and needs pins.
Victim 6- he has a severely broken lumbar, both of his crurals were broken in the femoral region, and his left brachial was crushed.
They all survived from their injuries.

We’re heading to a scene of an accident where a minivan has impacted a telephone poll doing 50mph. The victims are a 36 male, 32 female, 7 female, 14 male, and a 4 female. Once our ambulance gets near the scene there are several police vehicles and fire trucks around the area. The police set up a road block to not allow any disturbance while we try assessing the situation.
We sprint to the minivan where fireman gazes at a splinter from the telephone poll that stabbed into the 36 year old males’ neck. While I inspected the man’s body I notice several broken bones, his thigh, lower back, and arm, have all been broken or fractured. The 32 year old female sustained some serious injuries like a large shard of glass puncturing her armpit and through her chest. I’m assuming it has punctured her lungs as well. The kids surprisingly only had a few minor injuries. The 14 year old boy had a broken arm and hand. Then the 7 year old female had some scratches on her head and arms. The 4 year old girl only had a large bruise on her thigh.
36 Male – Sustained heavy damage to cervical region from a large splinter that broke off the Telephone pole. The splinter nearly entered up to the cephalic which is superior to the cervical region. The man also had broken bones in his femoral, lumbar, and brachial. The lumbar region had broken two vertebrates which made it hard to coordinate his thoracic and abdominal regions to remove the splinter from his cervical region. Thoracic and abdominal region are both inferior to the cervical region where the splinter punctured his cervical region. The rest of his injuries though were proximal from his cervical region.
32 Female – Has a large shard of glass deep puncture through her Axillary through her Thoracic. It punctured one of her pleural cavities which resulted in a punctured lung. It was ventral to her thoracic region. She also had some lateral damage to thoracic region from the entrĂ©e area of the glass shard.
14 Male – Had a broken brachial which is proximal to the broken manual.
7 Female – Her injuries were superficial, they were located on her cephalic and brachial regions.
4 Female – She also had a superficial injury, which was a bruise on her femoral region.

As the ambulance pulled up to the accident, I could see that a pick-up truck occupying two passengers had driven over a mini cooper occupying four others. Nothing was left of the small sports car; the same could be said for its driver. The 36 year old male driver was pronounced dead upon our arrival. The wheel of the truck had proven fatal on its track straight over the man’s head and crushed spinal cord. The passenger sitting behind the driver was in an unconscious state. The only other physical sign that she had been in an accident was the yellowish bruise ripening upon her left cheek. The woman sitting shot gun in the cooper was hysterical when I arrived at the scene of the accident. As we lifted her into the ambulance on a gurney, I was informed that she is/ was pregnant. However, the second wheel of the truck had gone through the passenger window of the cooper and could show to be fatal for her unborn fetus. Along with her pelvic injury, her hand was bleeding and crumpled in the most unnatural position. The child diagonal to the driver seemed to be in the best condition so far. He showed minor scratches on his chest and face.
As I went over to inspect the passengers of the pick-up truck, the other medics began informing me of their conditions. The driver of the truck seemed to suffer from a broken arm and scraped knee while his only passenger suffered critical conditions. The severity of her condition could not be determined on site.
All the patients were transported to the local hospital and are now being treated.
From the results of an autopsy report, the hospital was able to determine the driver’s cause of death. The Cephalic region of his skeletal system was crushed from the impact of the truck wheel. Once the brain was vulnerable, his nervous system went into shock and deteriorated. The second passenger who arrived to the hospital unconscious was treated for minor Cephalic lacerations, but is still in critical condition for he endures a concussion. The pregnant woman who was rushed to the emergency room arrived without child. Severe impact to the mother’s Pelvic Cavity caused damage to her Reproductive system and to the fetus’s pericardial and pleural cavities of the Thoracic region. Once at the hospital, it was determined that the mother suffered internal bleeding and an extreme distortion of her right Manus. The manual region could not be salvaged. The child, who was considered fortunate with a few scratches, arrived at the hospital in cardiac arrest. Apparently, he also endured internal damage to his anterior abdominal region.
The truck driver was treated for broken bones in the left sagittal planes of the Brachial and Crural regions. The proximal arm bone tore from the elbow and jammed into the shoulder. The trucks only passenger who arrived in critical condition suffered paralysis. A section of the patient’s spinal cord inferior to her lungs and superior to her Pelvic region was sliced from impact. Her nervous system went into failure.

I arrived on site to a car crash scene on Tuesday, January 25, 2011 at 9:23 pm at the intersection of State Road 14, Brick Church Road, and Six Corners Road. There were six victims at the scene and a total of three cars, two of the cars and hit head on and one car had the side of another car, it was a horrible site to see. Victims 1, 2, and 3, were in car one, victims 4 and 5 were in car two, and victim 6 was in her own car. Car one caused the accident, traveling south on State Road 14 car at high speed, car one swerved into car three’s lane, simultaneously car two slipped on a patch of ice coming out of Brick Church Road and slide into the middle of the intersection, it appeared that car three tried to slam on its brakes but slide and car one and car three sandwiched car two. Car one appeared to be under the influence of marijuana and further searching proved this to be true because there were multiple bags of marijuana found in the car, both cars two and three appeared to be completely sober.
Victim 1-Lucas Lament, age 16, driver, major concussion, cracked rips, bruising on the chest, and a shallow cut on the neck. Victim 2- Alex Dial, age 17, passenger, and broken nose, deep cut on the arm, and a fractured neck. Victim 3- Briana Lawson, age 16, passenger, suffered minor bruises and cuts. Victim 4- Andy Xavier, age 42, driver, fractured skull, broken ribs, broken femur, and deep cut around the abdominal. Victim 5-Lucy Xavier, age 13, passenger, fractured femur, broken neck, fractured back, bleeding from the chest, unconscious. Victim 6- Kallie Woodrow, age 30, driver, pregnant, broken arm, and a concussion.

Hospital Report:
• Victim 1- Lucas Lament, concussion, cracked rips, bruising in the thoracic region, and a superficial cut in the cervical region.
• Victim 2- Alex Dial, broken nose, deep cut in the brachial region, and a fracture in the cervical.
• Victim 3- Briana Lawson, suffered minor bruises and cuts.
